Arrival to Your Massage/Aesthetic Treatment 


Arrive for your appointment about 10 minutes prior to the scheduled starting time. All treatment sessions have a specific time schedule. If arrived late, your service may be shortened to keep on schedule and the original treatment time may be charged.

Cancellation Policy and Fees

 

Your therapy times are reserved. We keep your time exclusively for you. We value your business and ask that you respect the office scheduling policies. Give us a 24 hour in advance if you need to reschedule or cancel.

If the cancellation is within less than 24 hours, there will be a fee of $25 per person. The time of our clients and staff is valuable and have implemented this policy for this reason. When you miss an appointment with us, our therapists not only lose your business, but also the potential business of other clients who could have scheduled an appointment for the same time. Additionally, our staff is “on call” and have traveled to the office specifically for your service. For these reasons, we are obligated to compensate our staff for their time.

All services require a form of payment to guarantee a reservation. You will not be billed unless there is a cancellation or no show. Upon checkout, guests may choose their method of payment.

Late Arrival Policy

 

Late arrivals may not be honored or the treatment session may be cut short. If schedule allows, the treatment session may be honored partially or in full. The original reservation fee will be charged.

No Show Policy

 

At VIP Massage, we understand that unanticipated events occur in everyone’s life. However, we ask that you call if you cannot make it to your appointment. Clients who miss their appointments without giving any prior notification will be charged $25.
 

Informed Consent
 

The treatment plan will be discussed with you prior to each massage session. At your first visit with us you will be asked to sign the "New Client Information Forms" and/or “Intake VIP Massage Form for Outreach Events” treatment consent form stating that you have read and agree with the information, understand it, and agree to comply with the professional massage therapy.
 

Reservations
 

We recommend scheduling your appointment in advance to ensure availability. To accommodate your desired appointment a form of payment is required at the time of booking to guarantee your reservation. We hold this information as part of our cancellation policies and only make necessary charges to your account on the date of your scheduled service. Details of our cancellation policy are provided below.

*Please notify the receptionist of any allergies or sensitivities to aromas or ingredients before your appointment.
 

Scope of Practice

Licensed Massage Therapists do not diagnose or prescribe for medical conditions. We may refer you to a medical doctor for diagnosis or treatment. Massage should not replace the care of your physician.
 

Treating Minors
 

If the child is younger than 18 years old, we will ask a parent / guardian to sit in on the treatment.

Respect for Client Needs and Boundaries
 

Our Massage Therapists are happy to adjust pressure, temperature, musical volume, work longer on an area or move on if requested.

As the client, you may choose to: leave on as much clothing as needed for comfort, request specific massage methods or stop the massage at any time.

As the client you must agree to always be modestly draped. Only the area being massaged will be undraped. The clients will be kept informed of the area to be massaged.

Sexual interaction or discussion of any kind between the client and the massage therapist is never appropriate and will not be tolerated.



Auto Car Accidents

As a medical provider, we work with insurance companies after auto car accidents.

The massage therapy logs, that state the amount of units and heat, require your initials after or before your treatment. The Daily Notes and Claim Forms are sent directly to the insurance company.

The medical provider sets the price with the insurance company for the units. Auto injury claims accepted for insurance billing by units. Average 2-4 units. Therapeutic, Healing Procedure Treatments. Same rate for non-prescribed or prescribed therapeutic work. The prices are subject to change and may vary, $32-$59/unit  CPT codes 97124 massage, 97010 hot/cold packs. Chiropractic referrals welcome.

Client Confidentiality Notice
 

All client information and interactions are strictly confidential. Unless given prior authorization or consent, your visit and/or personal conversations will not be discussed or disclosed to anyone outside of our staff. The previous statement excludes the following personnel; your personal health doctor, your personal injury attorney or your medical insurance provider. Any inquiries, requests or paperwork regarding your visit will be transferred person to person. Any information that is not transferred in person requires confirmed consent, usually in the form of a signed authorization form from your power of attorney, doctor, medical/auto insurance representative, or you. As a medical provider, if this consent form is not present, then we reserve the right to contact your lawyer, medical provider, physical therapist, chiropractor with any further questions we may have. Upon request, you can receive your medical records by mail or fax.
 

Existing and New Medical Conditions
 

It is the responsibility of the client to keep the massage therapist informed of any medical treatment currently being taken, and to provide written permission from the physician, chiropractor, physical therapist, etc., that the massage may be continued.

The client must also keep the massage therapist informed of any changes in health conditions, personal information, address, phone number...ect.

For clients undergoing chemo and radiation therapies – Please note that we require a doctor’s note that states the doctor is aware of and agrees to the desired treatment.
 

Payment
 

Payment is full in due at the time services are rendered. Prices include sales tax. A 3.85% credit card fee will be added to your bill if you pay by credit card (over $199) for each $100 increase.
If after an auto car accident, the patient's insurance company will be charged directly.
